Meg and Jimmy kissed! Jimmy wakes up in his bed, processing what he just did. Alice enters to tell him all about her fun night with Sean and her friends. Jimmy tries to hold himself together in front of Alice.
Gaby is still seeing Maya, and things are looking up. Maya seems to be doing okay since the two of them played trivia together. However, Maya explains to Gaby that it’s very hard for her to make friends at her age and stage in her life. Gaby still encourages her to put herself out there, taking her on a little outing.
Liz and Derek stop by Jimmy’s house with a basket of muffins. What’s the catch? Liz asks Jimmy if he can talk to Matthew, who has been ignoring her because of what she said about him. Derek tells Jimmy that Matthew agreed to meet with a therapist, and he picked Jimmy. Liz then guilts Jimmy into helping them because she and Derek raised Alice for a year. Jimmy agrees. Jimmy then goes to the office, and Meg is there. Things are visibly awkward between them as Paul is also in the room. Once Paul leaves, Jimmy freaks out. Meg explains that she and her husband, Dave, have been separated for a while, but Paul doesn’t know. Jimmy still feels like a bad friend, but he is relieved in a way because he knows he’s able to put himself out there since Tia’s passing.
Brain, Ava, and Liz are playing with baby Sutton and taking pictures for a birth announcement. Brain shares that he never told his dad about the baby! Liz and Ava freak out and tell Brian that he needs to tell him. Brian is scared his dad won’t approve, as he’s been very judgmental of him his whole life. Brain begs Jimmy to help him with his dad, and Jimmy reluctantly agrees.

Jimmy sits down and talks to Brian about his dad. While Brain talks on and on, Jimmy calls Kip and hands the phone to Brian. After hilariously fighting over the phone, Brain comes clean and tells his dad about Sutton. Kip takes it very well after learning that he is a grandpa. This brings Brain joy.
Later, in the office, Gaby picks up on the weird energy between Meg and Jimmy. Jimmy hides their secret with the fact Meg is separated from her husband. Gaby completely comforts Meg as Jimmy meets with his next patient: Matthew. Matthew lays down on the couch and pretty much refuses to talk to Jimmy. Jimmy then asks why he agreed to meet with him. It turns out Liz and Derek paid him $20 to do it. Furious, Jimmy calls Liz and tells her that he will find another way to repay the both of them for Alice.
During Matthew’s therapy session, Liz is with Derek for an EKG when the nurse tells him that he needs to go to the hospital right away. Derek needs emergency surgery for one of his arteries. Thinking he won’t be okay, Derrick assures him that Dereks Don’t Die. Liz asks everyone to leave so her and Derek can have some time alone before he goes in. Matthew is still very upset, but Jimmy tells him that if he won’t be there for his mom, he will regret it for the rest of his life. Later at the hospital, Matthew shows up and comforts his mom at the hospital.

Alice invites Jimmy’s dad to her graduation. Jimmy is surprised, but then Alice reveals that she doesn’t want to look back and wish she had seen him more. Jimmy then tells her about how weird life is. It’s almost as if you have to see something bad happen before you can appreciate what you have.
In the end, Sean accepts the interview and we see Derrick has an engagement ring ready for Gaby. Meg is on the phone with Dave asking him to at least try counseling. Derek wakes up, surrounded by the ones that he loves. Dereks Don’t Die. The episode ends with Maya sitting in her living room with a bottle of wine. She is scrolling through pictures on Instagram, people are smiling and having fun without her. She calls Gaby to see what she’s doing, but Gaby tells her that she’s at the hospital visiting Derek. Maya hangs up and continues to drink.
This episode was unquestionably more on the emotional side. There were also very powerful aspects about life included throughout the dialogue which really worked. Meg and Jimmy’s relationship seems to be more on the one and done side, which is probably the best for all of them due to the fact that Paul is heavily involved in their lives. Paul’s move to Connecticut was surprisingly only brought up once, and the other characters don’t seem to know about it. Derek is truly an old soul and a beloved character by many, so for him to go through something this difficult was very heartbreaking. However, it was very heartwarming to witness all of the characters realize that they need to live their lives, and face their fears, no matter what other people think of them.
